Primary Purpose:           

Participate as a member of the District Transition Team and provide overall support and training to enhance students with disabilities transition from the high school setting to their post secondary setting. 

 

Qualifications:

Education/Certification:

Bachelor’s degree from an accredited university

Valid Texas teacher certificate with required special education endorsements

           

Experience:

Minimum 3 – 5 years successful experience as a special education teacher with  

extensive knowledge of federal, state, and local transition requirements for  

students with disabilities

Bilingual an asset

 

Major Responsibilities and Duties:

 

Instructional Management

    1. Assist CCISD students with disabilities to become contributing members of the community through employment, education, training, independent life skills and self-advocacy.
    2. Coordinate a set of activities for individual students focused on improving student academic and functional achievement in order to facilitate movement from school to adult life.
    3. Build relationships within the community to facilitate training and instruction in real environments.
    4. Assists school administrators, faculty, and other special service personnel in supporting students through transition into new education placements
    5. Participates in the ARD meetings when appropriate
    6. Offer accelerated instruction to each student who meets any of the state “at risk” criteria in order for the student to perform satisfactorily on the state assessment.

    1. Communicates effectively with students, professional staff, parents, and community in a positive manner.

School Improvement

    1. Provides consultation to meet the educational, vocational, and personal-social needs of students with special needs.
    2. Serves as a resource person for students and parents on problems affecting the educational, physical, emotional, and social growth of students with special needs.    

 Personnel Management 

    1. Works cooperatively with the staff to provide information concerning student needs.
    2. Works collaboratively with District Wide Transition Team members in developing and supporting students with special needs throughout the school district.        

Administration and Fiscal/Facilities Management

            1. Complies with district policies, as well as state and federal guidelines.        

 

Student management

              1.Provides research based training and support to campus staff, employers, and community in the area of supporting for students with special needs

              2. Assist in the development and implementation of data collection to support IEP goals and objectives for students with disabilities

              3. Observes students in school, community, and home as appropriate.

              4. Meets and consults with parents to help them independently implement strategies in the home and/or community.

              5. Consults with students, parents, and staff concerning issues related to programming for students with special needs

 

School/Community Relations 

    1. Collaborates with appropriate agencies, community groups, and school organizations as needed.
    2. Works cooperatively with and coordinates the use of community referral services as needed.
    3. Interacts constructively with a variety of professionals, age groups, cultures, families and socio-economic groups as needed.            

Professional Growth and Development 

    1. Participates in in-services/training programs.
    2. Attends workshops and participates in professional growth opportunities.
    3. Provides professional development as needed.

Other:

    1. Performs other duties as may be assigned by the Executive Director of Special Education

Mental Demand/Physical Demand/Environmental Factors:

 

Maintain emotional control under stress.  Frequent standing, stooping, bending, and kneeling, pushing, and pulling; regular heavy lifting of students.  Biological exposure to bacteria and communicable diseases.  Needs reliable transportation to work across the district.
